Just plop telomerase expressing rna into vacuoles and pump em into the bloodstream every few years? Not sure if that's a way to get rna into every tissue but seems, i'll check if there's studies on that.
Telomeres get fixed which as far as we can tell is the mechanism that causes cells to degrade with age.
Not much cancer risk because telomeres don't get replenished faster than a healthy cell will use them up.
